While you are at Clifton Hospital, the assessments that are done will help us to see what support you will need when you are discharged.
This may include equipment that you may need to help you to get safely around your home, adaptions to your home, and any care support that you may need. Some equipment may not be essential for discharge and will therefore be delivered once you are at home.
You will be included in the planning of your discharge and you should let the nursing staff and therapists know if you have any concerns about your discharge so these can be addressed as quickly as possible.
