Paediatrics Virtual Ward at a glance
Working collaboratively across Acute and Community Paediatric Services the Paediatric Virtual Ward has made a real difference to families across the Fylde Coast supporting early discharge and preventing hospital readmission. This has allowed families to stay at home together and children to be cared for in their own homes, whilst receiving excellent care from the Children’s Community Nursing Team, a positive difference for families.

End of Life/Palliative Care Virtual Ward at a glance
Trinity Hospice, in partnership with BTH, have launched a new service to allow a safe alternative to hospital or hospice for patients who need palliative or end-of-life care through community-based acute health care delivery.
The innovative service will see eligible patients admitted onto a ‘virtual ward’ where their observations can be monitored and medications quickly altered if their condition changes.
