Heart Failure Nurses

The aim of the Heart Failure Service is to improve the cardiovascular health of patients who have been diagnosed as having heart failure or heart failure syndrome. We offer comprehensive advice, education, and treatment in accordance with the National Institute for Health and Clinical Excellence (NICE) Guidelines for chronic heart failure. Primarily with patients who have experienced an episode of decompensating heart failure requiring an acute admission to hospital or deemed as unstable with heart failure symptoms to empower patients in self-management.

The heart failure nurse specialist team review patients throughout the hospital on the wards, following referral using the NEXUS e-referral system. Or automatically if NTproBNP is >2000.  Nurse led heart failure clinics also run daily in the outpatient setting within the LCC.  Referral to the NLHFC will be following hospitalisation, referral from other heart failure teams, HF ANP or from Consultant Cardiologists.
