Description

Under the Freedom of Information Act 2000, please can you provide me with the following:
The number of preventable or avoidable deaths for each of the last 5 calendar years (including 2019) broken down by hospital department (i.e. maternity/ accident and emergency) and condition (where known).
Preventable or avoidable deaths may be deemed as such by the hospital before an inquest or termed as such following its findings, please state what process the Trust followed to term the death as such.
