The Wyre Care Home Support Team, Poulton Community Midwives, Poulton District Nursing and Matrons services have relocated from Poulton Health Centre (Princess Avenue) to Poulton Civic Centre.
The new address is The Bungalow, Poulton Civic Centre, Breck Road, Poulton FY6 7PU.
Clinics currently located or delivered from Poulton Health Centre will be relocating over the next few weeks as detailed below:
Civic Centre clinics
Poulton Community Midwives: have commenced clinics at Poulton Civic Centre from Monday 24 May
Poulton District Nursing: clinics will move from Poulton Health Centre to Poulton Civic Centre from Tuesday 8 June
Anticoagulant Clinic: clinics will move from Poulton Health Centre to Poulton Civic Centre from Wednesday 9 June
Paediatric Continence: will recommence from Poulton Civic Centre on Thursday 8 July
The Podiatry Clinic, Memory Assessment Service and Minds Matter will recommence from Poulton Civic Centre (dates yet to be confirmed).
Other clinic moves:
Paediatric Speech and Language: clinics will commence from Thornton Health Centre (date yet to be confirmed)
Falls Service: clinics will recommence from Thornton Health Centre (date yet to be confirmed)
Paediatric Consultant: will recommence clinics at Thornton Health Centre and Poulton Civic Centre (date yet to be confirmed)
