Patient Information Notice – Transfer of Services

In October 2018, some of the NHS services provided by Blackpool Teaching Hospitals NHS Foundation Trust (BTH) will be transferred to the University Hospitals of Morecambe Bay NHS Foundation Trust (UHMB).

Services that are transferring relate to adults who are receiving care in the community including patients with long term conditions for example:

  • District Nursing
  • Continence service
  • Podiatry
  • Diabetes Specialist Nursing
  • Community MSK Physiotherapy

Included in the transfer are a number of children’s services for example:

  • Occupational Therapy and Physiotherapy
  • Speech and Language Therapy
  • Children’s Hearing
  • Children’s Community Nursing

Patients who are currently undergoing treatment will have their information transferred to UHMB.

It is expected that information relating to patients who have been recently discharged from one of the services affected (12 months previously) will also have their information transferred.

This is a clinical decision made in the interests of our patients. It will support the continuation of care because information will be readily available, helping to make the patient journey as seamless as possible. Information for patients that exceed the 12 month period will be shared with UHMB upon request should it be necessary for your care.

BTH and UHMB are working together to ensure the information is transferred safely and securely. Only information which is relevant and specific to the service will be transferred.

Upon transfer, UHMB will become the ‘Data Controller’ of your information meaning they will determine the means & purpose of any data processed.

BTH and UHMB will continue to manage the patient information in line with the General Data Protection Regulation (GDPR).
