Long-Term Physical Health Conditions

Having a long term physical health condition such as diabetes, COPD or other respiratory problems, or heart conditions, can affect all areas of our life not just our physical health. Supporting Minds is now providing a new service to help people with long term physical health conditions maintain the best possible health and wellbeing by offering psychological support alongside the support that you get from the physical health specialists, Nurses, health and wellbeing support workers, Occupational Therapists and / or your GP.

How can we help?

We offer a type of support called CBT, or Cognitive Behavioural Therapy. CBT looks at how our thoughts and behaviour affect the way we feel. Our aim is to give people tools with which to manage their mood and condition, in order for them to feel happier, healthier and more in control of life.

We also provide other therapies:

  • Mindfulness course
  • Counselling
  • Couples counselling for depression
  • Eye Movement Desensitisation and Reprocessing (EMDR)
  • Stress Control Course
  • Psychological Wellbeing Support

If you would like to access our service please use the Self-referral form or talk to the health professional that helps you to manage your health condition, and they can make a referral for you using the LTC Professional referral form.

Online Diabetes Course

Supporting Minds is now providing an online LTC diabetes support course. The course aims to help people with diabetes maintain the best possible health and wellbeing by offering psychological support alongside the support that you receive from physical health professionals.

The course consists of 6 sessions held on a weekly basis. Each weekly session lasts for 1 hour 30 minutes. The course will cover the following topics:

  • Getting started and understanding your problems
  • Anxiety
  • Depression
  • Challenging your thoughts
  • Problem solving and sleep
  • Relapse prevention
