How to Refer to SLT

Our outpatient service can be accessed by sending a referral form which is available from the department.  Those wishing to self-refer may use the same form.

Please ensure that the client meets our referral criteria and that the form is completed fully. Incomplete referrals will be returned causing a delay in clients being contacted.

All inpatient referrals are dealt with internally by the E-Referral system.

Processing Incoming Referrals

Our service accepts referrals for people aged 18 years and over.

Initially, all referrals are screened to see whether they meet our referral criteria and contain sufficient information. They are then prioritised, and an appointment offered.

Inpatient referrals may be offered follow up in the community once discharged from hospital. This is to ensure continuity of care and enhance the patient experience.

Domiciliary Visits

A domiciliary service is only available for clients who are confined to their homes. Visits will only be undertaken where absolutely necessary. Advice may be given over the telephone.
