The service is delivered from the Royal Lancaster Infirmary Monday – Sunday.
The REACT service supports adults over 18 registered with a GP in Lancaster, Morecambe, Carnforth and Garstang areas.
The team consists of staff with advanced assessment skills who are Nurses, Physiotherapists, Occupational Therapists or Social Workers.
REACT has been developed to assess people to prevent avoidable admission to hospital and to help people leave hospital within 48 hours following admission.
REACT staff will assess people within the Accident and Emergency department and assessment wards at Royal Lancaster Infirmary (RLI) GPs, community health and social care professionals also refer to REACT to request a telephone assessment for a person who is at home to prevent them being admitted to hospital. In this case the REACT team will give telephone advice and may request community services to support patients at home.
