Together with St John’s Hospice this integrated team provide a specialist service for adult patients living with life-limiting conditions. The team provides education, clinical care, psychological and emotional support to people with end of life care needs when their condition requires specialist intervention, the team interface with both GP, hospice and hospital-based colleagues.
The service also provides education and support for staff involved in end of life care.
