Heart Failure Occupational Therapist

The role of the Heart Failure Occupational Therapist is to work closely with the Heart Failure Nurses, Advanced Practitioner, Consultants and Device Nurses providing holistic care and assessments in both the acute and community settings delivering practical support to facilitate recovery/barriers that prevent people completing Occupations.

Fatigue management, provision of equipment and advice & education is completed to help reduce exacerbations of symptoms, promote independence, and increase health and wellbeing. Consideration is given to all aspects of physical, psychological, environmental, and social needs. This benefits patients with self-management of their Heart Failure symptoms, supports, gives reassurance in turn increases quality of life, reducing and preventing hospital admissions, improving patient outcomes and experience.
