What does the project provide?
Blackpool Teaching Hospitals NHS Foundation Trust and Macmillan Cancer Support have come together to address the cancer care challenges faced by patients and their carers. They have created a ‘Living With and Beyond Cancer Project’ which aims to improve cancer services to meet the needs of patients.
This will entail implementing “The Recovery Package” (see diagram below)
The Recovery Package is a series of key interventions which, when delivered together, can greatly improve outcomes for people living with and beyond cancer. This enables individualised follow-up care either through a supported self-management approach, with rapid access back into the specialist team should this be needed, or continued face-to-face follow up with health care professionals.
