Self-referral form
Self-referral is for anyone aged 16 or over and registered with a Blackpool or Cleveleys GP.
Blackpool Talking Therapies does not offer 24-hour or weekend access, crisis or urgent treatment, treatment for alcohol or drug problems or treatment for severe and enduring mental illness. Please discuss these needs with your GP.

Once registered you will be contacted to arrange an appointment for a brief welcome and assessment call with one of our Psychological Wellbeing Practitioners. If you think that you will struggle speaking on the phone, please let us know when we contact you to discuss an alternative appointment.
LTC Professional Referral form
Around 40% of people with low mood and anxiety also have a long-term physical health condition. If you are a health professional and you would like to make a referral, we have trained clinicians who work closely with our physical health colleagues to ensure the physical and mental health aspects of living with a long-term physical health condition are worked on together.
Please only complete this form if you are a health professional making a referral for a patient with a diagnosed long term physical health condition. Alternatively, you can download and complete this form. (Please note, this form may not meet accessibility standards)
Guided Self-referral
If you are a professional supporting someone, and your client would benefit from our service but requires support to self-refer, you can complete a guided self-referral together.
Once you have discussed a referral into Blackpool Talking Therapies service, you can complete a guided self-referral in two ways;
- By phone: Call 01253 955700 when you are together. The client must be present and confirm over the phone that they consent to the referral.
- Online: you can offer to fill in our self-referral form online together with your client.
