What we do

The team offer the following services to housebound patients who are not on the caseload of other Community Nursing teams.

  • Phlebotomy – North Lancashire only
  • Blood Pressure recording
  • Urinalysis – North Lancashire only
  • COPD Annual Reviews
  • ECG recording (planned)
  • Diabetic Annual Reviews
  • CHD Annual Reviews
  • COPD Annual Reviews

Referral into the service is via the patient’s GP practice.

This team cover home visits for the whole of the North section of North Lancashire (i.e all patients under the Lancaster, Morecambe, Carnforth, and Garstang Clinical Commissioning Group)
