The Family Nurse Partnership (FNP), is a structured programme of intensive home visits delivered by highly trained Family Nurses to young mums and their partners who are pregnant with their first baby, continuing up to the baby’s second birthday.
Our aim is to work with our young mums to ensure they have a healthy pregnancy, to improve the child's health and development, and help plan for the future and achieve aspirations.
Note: The Family Nurse Partnership (FNP) is no longer taking referrals.
