Extensive Care

The Extensive Care Service is based around the patient’s individual health and social needs and provides one dedicated team of professionals who will come together to support people to remain at home.  This service is designed to benefit people who are aged 50 or over and have two or more physical long term conditions, which can include dementia or frailty (for frailty patients should be scored either using the Rockwood Frailty Score or EFI)

Care is centred on providing patients with a single point of contact with one team looking after their needs and providing them with more joined up care rather than a variety of different services looking at each long term condition separately.


Extensive Care accepts referrals from a number of health care professionals across the Fylde coast. When invited to an initial assessment, patients will be offered the opportunity to attend a number of sites these include:

  • Lytham Primary Care Centre and South Shore Primary Care Centre (South Locality)
  • Moor Park Health and Leisure Centre and Poulton Civic Centre (North Locality)
