The Extensive Care Service is a community based service which supports people to live well alongside their existing long term conditions.
The service has been developed for people aged 18 years and over who have at least one long term health condition. These may include diabetes, a heart or lung condition, or a variety of other chronic health conditions which can include dementia or frailty (for frailty patients should be scored either using the Rockwood frailty Score or EFI)
The Extensive Care Service consists of a multi-disciplinary team of health professionals including doctors, nurses and a pharmacy team. The service has close links with other community services including GP’s and other hospital based services.
The Extensive Care Service accepts referrals from health care professionals across the Fylde coast. To make a referral over the telephone please call 01253 951400 between the hours of 9am to 5pm Monday to Friday.
