Dementia Strategy 2016-19

‘Remember the ME in deMEntia 2016-2019’

was developed after engagement work with staff, carers, patients, local  health economy partner organisations, and voluntary services to inform agreed key priority action areas for  the next three years.

A large number of improvements have already been facilitated to ensure care for patients and carers continually improves according to best practice guidelines.

Some of those improvements to date are:

Identification of a lead clinician for dementia care

Development and implementation of our Dementia Friends campaign

Development of our Dementia Champions Role

Implementation of the Butterfly Scheme

Comfort boxes on each ward to support cognition

Paint Me A Picture initiative – to help staff ‘know’ the patient behind the dementia, their likes, dislikes

The Dementia Corridor – 72 metres of corridor with memory triggering scenes and smells

Collaborative working with the Blackpool Carers’ Centre to support carers of people with dementia

Collaborative working with the social enterprise ‘ Pictures to Share’

Use of the National Dementia Screening Tool for all patients over 75 years of age admitted to the hospital

Registration in the RCP National Audit of Dementia Care and completion of environmental Care Audit

Utilisation of rem pods and reminiscence in clinical environments

Facilitation of dementia awareness sessions and other targeted training sessions

Signing up to support the national John’s Campaign

Refurbishment of wards at Clifton Hospital in line with Enhancing the Healing Environment ethos

Clifton Hospital carers open days and Clifton dementia hub

Creation of a dementia friendly garden at Clifton Hospital

The Dementia Advisory Board comprises of

  • A medical Consultant
  • Dementia Leads from all divisions
  • Clifton Hospital lead
  • A governor
  • Director of nursing
  • Deputy Director of nursing
  • Communications representative
  • Estates representative
  • Patient/relation representative

The role and purpose of the group is to drive and coordinate a programme of improvements based upon the delivery of integrated care and support for people with dementia across health and social care.  The guiding principle will be working together to secure required outcomes for people with dementia and their families and carers.

The group will:

  • Provide leadership and coordination in implementing the dementia strategy
  • Define and lead the agenda for implementation to be worked upon by varying dementia work streams
  • Identify and agree priorities and actions with key stakeholders, commissioners and providers to ensure implementation
  • Monitor the completion of objectives as identified within the strategy
  • Ensure that the engagement with people with dementia and their families and carers is a consistent part of the process of implementation
  • Agree and monitor funding proposals for items relevant to the care of people with dementia
  • Invite cooperation and seek to move forwards through cooperation.

 

The group feeds into the Quality Committee by exception.
