Rapid Response, Multi disciplinary Team (MDT)
Rapid Response Team South is a nurse led, physio and occupational therapy service that prevents hospital admissions and facilitates early hospital discharge of patients who meet specific criteria with a clinical need. Patients are required to have been seen by the referring personnel in the previous 24 hours. The service aims to provide an alternative care option to hospital.
The COPD ESD is part of the Rapid Response service which facilitates partnership working between Blackpool Teaching Hospitals NHS Foundation Trust, North Lancashire Primary Care Trust and other health and social partners in order to provide an integrated approach to the management of patients with COPD in the home environment. The COPD home management pathway is supported by a team of skilled staff from within the Fylde and Wyre rapid response team. Referrals are assessed by a led respiratory consultant and respiratory nurse at Blackpool Victoria Hospital who follow the patient up in clinic six weeks post discharge from ESD.

Patients who wish to be referred to the Rapid Response Team, need to contact their GP and request a referral.
