Extensive Care

The Extensive Care Service provides a different way of delivering patient care. It is based around supporting people to live well alongside their existing long-term conditions. It provides one dedicated team of multi-disciplinary professionals, including Consultant Geriatrician, Extensivist GP’s, Nurse Consultant, Advanced Practitioners, Community Matron, Clinical Care Co-ordinators, Pharmacists, Medical Optimisation Technicians and Assistant Practitioners supported by an Administration Team, who will come together to provide them with the best package of care.

This service is designed to benefit people who are aged 18 or over and have one or more physical long term conditions, which can include dementia, diabetes or frailty (for frailty, patients should be scored using the Rockwood frailty Score).

In addition to providing health support, the service has close links with a variety of other community and hospital services and keeps in close contact with the patient’s GP. As well as providing clinical support, the service aims to help with patient’s wellbeing needs. The team is able to refer onto health and wellbeing support workers and work closely alongside them within the existing community teams. The team will develop a complete plan of care for the patient and will ensure they are fully informed about their health conditions.

Once a referral has been received, the service will contact the patient to organise an initial assessment which consists of a full comprehensive assessment either in clinics based across the Fylde Coast or in the patient’s home. This will give the team the opportunity to explore the patients’ needs and create an individualised plan of care.

The Extensive Care Service covers Blackpool, Fylde and Wyre and runs clinics out of Lytham Primary Care Centre, South Shore Primary Care Centre, Moor Park Health & Leisure Centre, Poulton Civic Centre and Fleetwood Hospital.
