Extensive Care

The Extensive Care Service is a different way of delivering your care. It is based around the patient’s individual health and social needs and provides one dedicated team of professionals who will come together to provide them with the best package of care.

This service is designed to benefit people who are aged 50 or over and have 2 or more physical long term conditions, which can include dementia or frailty (for frailty patients should be scored either using the Rockwood frailty Score or EFI)

Care is centred around providing patients with a single point of contact with one team looking after their needs and providing them with more joined up care rather than a variety of different services looking at each long term condition separately. The team will develop a complete package of care for the patient, led by expert healthcare professionals, including an Extensivist Doctor, Advanced Practitioner and Clinical Care Coordinator.

Once a request for assessment has been received the service will contact the patient to invite them to an initial assessment with an Extensivist Consultant/GP that will give us the opportunity to get to know them and to work on creating an individualised care plan which will clearly set out how the service will benefit the patient.
