The team provide a community service for patients requiring diagnosis or living predominantly with heart failure or arrhythmia. They support patients to remain well and self-manage where possible.
The team refer patients onto the local acute hospital or specialist colleagues in Blackpool when specialist consultations or procedures are required. The service operates Monday to Friday only.
