Lessons Learnt is a HENW patient safety training programme, which forms part of the formal Foundation Programme teaching sessions over six dates in each year.
Purpose
It allows trainees to discuss and learn from patient safety incidents encountered in the workplace - within a safe, facilitated forum.
Format
Trainees volunteer to present incidents observed by or involving a trainee that could have or did lead to patient harm. Sessions are held under Chatham House Rule to encourage open debate and discussion and attendees are ‘free to share information arising from the session, but the speaker identity should not be revealed’.

Session Facilitators
Senior doctors (consultants/ senior specialty trainees) trained in incident analysis facilitate the sessions and escalate serious concerns as appropriate according to GMC guidance.
Session Leads
Each year, we look for 3-4 FY1s to train up as Leads to run the six dedicated 60-minute sessions built into your local Foundation Teaching Programme.
