New Starter Form

  • Use of this form is for new starters into the Trust only.
  • This form is not for processing transfers from one department to another.
  • The form requires completion by both the manager and the new starter at the pre employment meet.
  • On completion and submission to Recruitment(for pre-employment checks) the form is then returned to the hiring manager for agreement, and confirmation of your start date.
  • Workforce Information are subsequently notified upon receipt, and the form is finally sent to Payroll for input.
