Assignment Change Form

Use the Assignment Change Form for the following:

  • Promotions
  • Transfers between departments *
  • Hours Change
  • Acting up
  • Additional Posts
  • Return from maternity/paternity/adoption
  • Any other change that will impact on pay, post or budget

*When an employee is transferring between departments it is the responsibility of the receiving manager (in the new department) to complete the form as soon as the date of transfer is known.

Once submitted, your finance manager will approve or reject it.

On approval it goes directly to Payroll for action.

If rejected the submitting manager will receive an email informing of the rejection.
