Information Breach

Please find below the information you need in relation to the letter sent to all staff on 6th May 2015 concerning Personal Data.

The Trust has made provision for all employees to be registered with the Cifas Protective Registration Service.  This service offers protection against identity theft and they will monitor any activity and alert you.

If you wish to use this service, we require you to opt in.  You can do so by completing the Cifas Opt in Form, available by clicking the link here: Cifas Opt In form

Then send via email to the CifasOptin email address at: or by post to:

Blackpool Teaching Hospitals
Whinney Heys Road

The HR team will then register you with theCifas service which will be for a period of 12 months. Should you want your details removing fromCifas before the 12 month period has ended, please email theCifasOptin email address requesting that your registration be removed.

If you are emailing us to request a copy of your information you must do so from your work email address and include the following information:

  • Name
  • Date of Birth
  • National Insurance Number
  • Payroll number - from your payslip

We will only be able to respond to you via your work email address.

The Trust collects Equality and Diversity data on behalf of the government for employment monitoring purposes.  The Trust therefore relies on employees providing this information to us.  It is appreciated that as a consequence of this breach you may wish to change some of your data; however we would ask that you consider this carefully before taking this step.

If after careful consideration you still wish to change any of your information stored by the Trust, please follow the links below stating what data you want amending.
