Description

1). I would like to know how many times patient notes have been lost either permanently or temporarily for each month of 2019.

2). I would like to know how much money the trust has paid out in compensation claims to patients where a loss of clinical information has occurred or contributed to delayed or inadequate patient care.

3). I would like to know the current average time period between clinic letter dictation and subsequent posting to GP/patient for adult and paediatric medical directorates.

4). I would like to know the total numbers of outstanding delayed or unfinished discharge summaries from acute general medicine for each month of 2019.

2). We do not hold this information as the Trust do not pay compensation, it is paid by the NHS Litigation Authority. You may contact them on the following link:

We do not collate this information in a manner that is easily accessible.  The information is not held centrally but individually by divisions and departments and would require a manual trawl through paper records which would constitute disproportionate effort. Therefore this information is therefore exempt on the grounds of cost (Section 12(1)).
