In addition to the answers provided in the attached document, further information that may be of interest is below;
- Those patients that are assessed by Social services and/or Age UK are likely to have discussions around provision of food and such.
- There is a pilot patient information leaflet being rolled out at our Trust containing a section for the patient, which prompts a series of questions in readiness for discharge. These include questions such as ‘do you have your keys, do you have suitable outdoor clothing, have you transport, etc.
