Election to Council of Governors

1. Nomination forms can be obtained from the Returning Officer from Monday, 10^th July 2017, between the hours of 9 am to 5 pm:-

• via email at:- members@bfwhospitals.nhs.uk;
• via telephoning:- the Membership Office on 01253 956673;
• via post from:- The Returning Officer, C/o: Corporate Assurance, 1st Floor Main Entrance, Blackpool Teaching Hospitals NHS Foundation Trust, Whinney Heys Road, Blackpool, FY3 8NR.

2. Completed nomination forms must be returned to the Returning Officer by 5 pm on Wednesday, 26^th July 2017:-

• via email at:- members@bfwhospitals.nhs.uk;
• via post from:- the above address.

3. Should any nominee wish to withdraw their nomination, they must submit this in writing to the Returning Officer by 5pm on Monday, 31^st July 2017.
4. Ballot papers will be distributed to qualifying members on Monday, 14^th August 2017.
5. Completed ballot papers must be received by the Returning Officer, at the above address, by 5 pm on Tuesday, 5^th September 2017.
6. Ballot boxes will be available at the following sites:-

• Queen Victoria Centre, Thornton Road, Morecambe, LA4 5NN
• Gillow District Nursing Offices, Slyne Road, Lancaster, LA1 2HT
• Garstang Medical Centre, Kepple Lane, Garstang, PR3 1PB
• Clifton Hospital, Pershore Road, Lytham St Annes, FY8 1PB
• Lytham Primary Care Centre, Warton Street, Lytham, FY8 5EE
• Main Reception, Blackpool Teaching Hospitals, Whinney Heys Road, Blackpool, FY3 8NR.
